Starting a new medication can be confusing, especially when it involves learning the right dosage and how to properly administer it. This is often the case with Ozempic, an injectable medication for managing type 2 diabetes and promoting weight loss. The good news is that once you understand the dosing guidelines and injection technique, taking Ozempic can become second nature.

In this article, we will walk through Ozempic dosages, including the levels that lead to weight loss benefits. We will also cover steps for using your Ozempic pen, along with tips for making sure the medication works optimally for you.

Ozempic Dosages

Ozempic is a pre-filled injectable pen that comes in four dosages:

  • 25 mg
  • 5 mg
  • 1 mg
  • 2 mg 1

Each Ozempic pen lasts one month and contains enough medication for four doses, with pens coming in three sizes based on the dosage:

  • 2 mg/1.5 mL pen: Delivers four doses of 0.25 mg and two doses of 0.5 mg or four doses of 0.5 mg.
  • 4 mg/3 mL pen: Delivers four weekly doses of 1 mg.
  • 8 mg/3 mL pen: Delivers four weekly doses of 2 mg. 2

Ozempic is injected once per week, subcutaneously - meaning right under the skin in your abdomen, thigh or upper arm. It can be taken at any time of day, with or without food. 1

When you start Ozempic, your doctor will prescribe you a low dose and gradually increase it over time. This slow introduction allows your body to adjust to the medication. 1 Typically, your doctor will have you follow this schedule:

  • Month 1: You will inject 0.25 mg once weekly for four weeks. This low dose helps your body get accustomed to Ozempic.
  • Month 2: You will inject 0.5 mg once weekly for four weeks.
  • Month 3: If your blood sugar levels are still high, your doctor will increase your prescription to 1 mg once weekly.
  • Month 4: If the 1 mg dose is ineffective at controlling your blood sugar levels, your doctor will increase your dose to 2 mg once weekly. 1

Ozempic Dosage for Weight Loss

Weight loss is a common side effect of Ozempic treatment , but it takes time to build up to the higher doses that tend to have the greatest impact on weight. When starting Ozempic, the dose is gradually increased over several months to minimize side effects.

When you first start Ozempic, you begin at a low dose of 0.25 mg once weekly for four weeks. This low dose allows the body to adjust to the medication without significantly lowering blood sugar or promoting weight loss. 1

After four weeks, your doctor will increase your dose to 0.5 mg once weekly. 1 In clinical trials, patients taking 0.5 mg of Ozempic lost an average of 8.2 pounds over 30 weeks. 3

More significant weight loss and blood sugar improvements are typically seen at higher doses. In one 40-week study, people taking 1 mg per week lost about 13.2 lbs on average. And those receiving the maximum 2 mg dose lost approximately 15.2 lbs on average. 4

How to Take Ozempic

If you are a new Ozempic user, your doctor will guide you through the process of using the pen. Each pen comes pre-filled and is accompanied by NovoFine Plus 32G 4 mm disposable needles. 2

To ensure that you are administering the medication correctly, follow these steps:

  1. Wash your hands with soap and water.
  1. Take a new needle and remove the paper tab.
  1. Push the needle straight onto the pen and turn it until it is securely in place.
  1. The needle is covered in two caps that need to be removed. First, pull off the outer needle cap and set it to the side. Next, pull off the inner needle cap and throw it away.
  1. Turn the dose selector at the end of the pen to your prescribed dose (0.25 mg, 0.5 mg, 1 mg, or 2 mg). You will hear it click with each turn. If you select the wrong dose, simply turn the selector forward or backward to the proper dose.
  1. Choose an injection site: front of your thighs (towards the outer side), the front of your abdomen (2 inches away from your belly button), or on the outer surface of your upper arms (3 inches below your shoulder and 3 inches above your elbow). Remember to rotate the injection site each time you inject.
  1. Wipe the injection with an alcohol swab and let it dry before injecting.
  1. Insert the needle directly into your skin at a 90-degree angle. Make sure you can see the dose counter throughout this process. If you cover the dose counter with your fingers, it could stop the injection.
  1. Press and hold the dose button until the counter shows “0” ¾ you may hear a click. Keep holding the dose button for 6 seconds, then remove the needle. Once the needle is removed, you can stop pressing the dose counter.
  1. If bleeding occurs, apply light pressure with gauze.
  1. Carefully put the needle back into the outer cap, then dispose of it in a sharps container or a heavy-duty plastic container. 2

Missing an Ozempic Dose

If you miss a dose of Ozempic, take it as soon as you remember within five days. 2 For example, if you usually take your dose on Tuesday morning but realize you forgot on Thursday afternoon, go ahead and take it then.

However, if more than five days have passed since you missed your dose, skip the missed dose and take your next dose on your regularly scheduled day. 2

Tips For Success

Follow these tips to optimize Ozempic’s effectiveness:

  • In addition to taking Ozempic, it’s important to maintain a healthy lifestyle. Eating nutritious foods and getting regular exercise can aid in weight loss and diabetes management. The medication works best when combined with positive lifestyle choices. 1
  • If you inject insulin along with Ozempic, be mindful of the injection sites. Ozempic and insulin can be injected in the same general body region, such as the abdomen, but make sure the two injections are not right next to each other. 2
  • Proper storage is key for keeping Ozempic effective. Store unopened, unused Ozempic pens in the refrigerator between 36-46°F. Once you start using a pen, you can keep it at room temperature between 59-86°F or in the fridge between 36-46°F. 2
  • Carefully monitor yourself for any new or worsening symptoms after starting Ozempic. Contact your doctor right away if you notice anything unusual or bothersome. They may need to adjust or change your treatment plan. 1

Conclusion

Ozempic shows promise as an effective tool for managing type 2 diabetes and aiding in weight loss efforts. By improving blood sugar control and reducing cardiovascular risks, Ozempic can be a helpful addition to a comprehensive diabetes treatment plan.

The weight loss benefits that many patients experience while taking Ozempic have also prompted some doctors to prescribe it off-label for weight management. While not yet FDA-approved for this purpose, Ozempic's effects on appetite and calorie absorption may complement lifestyle changes like diet and exercise for some individuals struggling with excess weight.
